    Overview 

    This 1.5 hour online module equips healthcare professionals in hospital settings with the knowledge, skills, and confidence to effectively integrate alcohol use disorder (AUD) treatment into clinical practice. Topics include addressing stigma, recognizing AUD as a chronic disease, understanding available pharmacological treatments, identifying appropriate medication options for co-occurring clinical conditions, managing potential medication side effects, and fostering a multidisciplinary approach to care that is patient-centered and enhances patient outcomes. Ultimately, the module aims to improve the quality of care for individuals with AUD by promoting evidence-based strategies within hospital settings, supporting better treatment and recovery outcomes, and reducing the long-term impacts of alcohol use.

    The target audience for this activity includes all clinical healthcare professionals in hospital settings.

    This activity addresses the following ACGME Competencies: Patient Care and Procedural Skills; Medical Knowledge and Interpersonal and Communication Skills, and Systems-based practice.

    Learning Objectives

    Upon completion, learners will be able to: 

    1. Describe the epidemiology of alcohol use disorder (AUD) in hospital settings.
    2. Examine the stigma surrounding AUD, as well as the challenges and benefits of integrating AUD treatment into hospital-based care.
    3. Determine the importance of universal screening in detecting alcohol use disorder.
    4. Identify the clinical manifestations of alcohol withdrawal syndrome (AWS), assess risk for and severity of AWS, and manage AWS and post-acute withdrawal syndrome (PAWS).
    5. Demonstrate ability to effectively prescribe and manage FDA-approved Medications for AUD (MAUD).
    6. Determine the appropriate management of co-occurring conditions with AUD and the appropriate timing for initiating MAUD.
    7. Explain the role of psychosocial treatment in managing AUD.
    8. Develop effective transitional care plans and post-hospital linkages for patients with AUD to ensure continuity of care and support recovery.

    University of Colorado School of Medicine

    Susan L. Calcaterra, MD, MPH is an Assistant Professor of Medicine at the University of Colorado, Department of Medicine, Division of General Internal Medicine. She is the Director of the Addiction Medicine Consultation service at the University of Colorado Hospital. Her areas of research include the expansion of hospital-based addiction treatment, opioid stewardship in the hospital setting, and hospital-based interventions to reduce opioid overdose deaths post hospital discharge.

    Oregon Health & Science University

    Dr. Sokolski specializes in the treatment of people with substance use disorders and psychiatric disorders. She chose these areas of focus after recognizing the need for increased access to both addiction and mental health services and the often-intertwined natures of these conditions. She enjoys building relationships with her patients to understand their experience and empower them in their recovery.     Rebecca Payne, MD

    Dr. Payne completed psychiatry residency and an addiction psychiatry fellowship at the Medical University of South Carolina (MUSC).  She practiced in a variety of settings as an Associate Professor at the University of South Carolina School of Medicine Department of Neuropsychiatry and Behavioral Science prior to assuming her role as the Medical Director of the South Carolina Recovering Professional Program (SCRPP) in 2020. She is board-certified in psychiatry, addiction psychiatry, and forensic psychiatry and is an active member of the American Academy of Addiction Psychiatry.

    Vanessa Loukas, MSN, FNP-C, CARN-AP

    Clinical Nurse Educator

    Grayken Center for Addiction Training and Technical Assistance at Boston Medical Center

    Vanessa is a clinical nurse educator for Boston Medical Center's Grayken Center for Addiction Training and Technical Assistance, an assistant professor of medicine at Boston University Chobanian & Avedisian School of Medicine, and a nurse practitioner at Boston Medical Center. She has expertise in managing substance use disorders, co-occurring psychiatric conditions, and infectious diseases in primary care settings. Her clinical interests include harm reduction, low-barrier treatment, medical addiction groups, and improving care for persons involved in the criminal-legal system. Through her primary role on the Grayken TTA team, Vanessa contributes to peer-reviewed publications, evidence-based clinical guidelines, development and delivery of continuing education programs, and other resources for providers supporting patients with substance use disorders. Prior to joining the Grayken TTA team, Vanessa practiced as a nurse practitioner and was the program director for the addiction treatment program at the South End Community Health Center, caring for high-risk patients with substance use disorders and psychiatric diagnoses. Her work integrated on-demand treatment, infectious disease treatment, and medical addiction groups including within the male/female re-entry/recovery units at the Suffolk County House of Corrections. Vanessa received her Master of Science in Nursing (MSN) from Simmons University and is a Certified Addiction Registered Nurse – Advanced Practice (CARN-AP) through the Addictions Nursing Certification Board (ANCB).

    Gene Im, MD, FAASLD

    Dr. Gene Y Im is an Assistant Professor of Medicine in the Division of Liver Diseases at the Icahn School of Medicine at Mount Sinai. He earned his B.A. from the University of California, Berkeley, graduating with honors. He completed his Internal Medicine residency and fellowship in Gastroenterology at Winthrop-University Hospital, the primary teaching affiliate for the SUNY Stony Brook School of Medicine. He then completed an advanced fellowship in Transplant Hepatology at the Mount Sinai Medical Center. He is currently the Associate Program Director of the Transplant Hepatology fellowship and RSS course director of the Division of Liver Diseases at Mount Sinai.

    Daniel L. Dickerson, D.O., M.P.H.

    Daniel Dickerson, D.O., M.P.H., is Associate Research Psychiatrist at UCLA, Integrated Substance Abuse Programs (ISAP). His research focuses on the development of substance use treatment and prevention programs for American Indian/Alaska Native youth and adults. He is currently co-Principal Investigator on National Institutes of Health-funded study, "Motivational Interviewing and Culture for Urban Native American Youth (MICUNAY)" an alcohol and drug use prevention research program targeting urban Native American youth. He is also Principal Investigator on a study investigating the potential benefits of a substance use intervention utilizing drumming for Native Americans, "Drum-Assisted Recovery Therapy for Native Americans (DARTNA)," and is also co-Principal Investigator on "Native American Youth Sleep, Health, and Wellness (NAYSHAW)," a study analyzing health and sleep among urban Native American youth.

    Autumn Ambroday, DO

    Dr. Autumn Ambroday is board certified as a psychiatrist and addiction medicine physician. She is currently working as the Behavioral Health Medical Director for White Earth Nation and oversees the Tribal MAT and Mental Health programs. Dr. Ambroday received her medical degree from A.T. Still University Kirksville College of Osteopathic Medicine and completed psychiatry residency at the University of Arizona with additional training in addiction. She specializes in dual diagnosis patients, trauma informed care, and telehealth with rural, underserved areas.

    Overview 

    This module provides an in-depth exploration of alcohol use disorder (AUD) and benzodiazepine use disorder (BUD). It focuses on diagnostic criteria, prevalence, and the substantial impact these disorders have on health and mortality rates, including accident-related fatalities. Learners will analyze the complexities of co-occurring AUD and BUD, particularly during intoxication and withdrawal, and will learn to identify high-risk situations associated with these disorders. They will utilize effective screening tools to assess AUD and BUD, determine appropriate levels of care, and implement evidence-based interventions. The module also covers an analysis of medications used for AUD and BUD, strategies for withdrawal management, and the development of return to use plans with a focus on strategies to reduce the risk of harm. 

    The target audience for this activity includes all clinical healthcare professionals.

    Learning Objectives

    Upon completion, learners will be able to: 

    1. Differentiate between benzodiazepine use and benzodiazepine use disorder.
    2. Assess and manage the complexities of patients with both AUD and benzodiazepine use/use disorder (BUD).
    3. Identify the risks associated with concomitant AUD and benzodiazepine use/BUD during intoxication, withdrawal, and in high-risk situations.

    Deanna Bridge Najera, MPAS, MS, PA-C, DFAAPA

    Deanna Bridge Najera completed her PA education at Chatham University in Pittsburgh. She attended the University of Maryland to earn her undergraduate degree and also obtained a master’s degree in Clinical Mental Health Counseling from Shippensburg University. She currently splits her time between a suburban emergency department and a county health department. Additionally, she provides limited outpatient psychiatric services at a community mental health agency and a local college. She is a regular speaker on the state and national level and has served in multiple leadership capacities.

    Emily Brunner, MD, DFASAM

    Emily Brunner is board-certified in family medicine and a distinguished fellow in the field of addiction medicine. She has experience treating addiction in both inpatient and outpatient settings. She is an experienced physician and specializes in trauma-informed clinical treatment of substance use disorders with a comprehensive and compassionate approach. Dr. Brunner has been involved in the leadership of the Minnesota Society of Addiction Medicine and is currently a member of the national board of the American Society of Addiction Medicine. She also serves on both the methodology and legislative advocacy subcommittees.

    Paxton Bach, MD, MSc, ABIM, FRCPC, FASAM

    Paxton Bach is a Clinical Assistant Professor in the Department of Medicine at the University of British Columbia and a general internist and addiction medicine physician at St. Paul’s Hospital in Vancouver, BC. He also serves as the Co-Medical Director for the British Columbia Centre on Substance Use, and is the Director of the BCCSU Clinical Addiction Medicine Fellowship Program. Dr. Bach is certified as a specialist in General Internal Medicine through the Royal College of Physicians and Surgeons of Canada, and an Addiction Medicine specialist through the International Society of Addiction Medicine and the American Board of Preventive Medicine.

    3. Review the range of differential diagnosis, which includes both acute and long-term conditions related to AUD, such as withdrawal, delirium from other causes, and other medical or mental health conditions.
    4. Recognize the effectiveness of medications, the appropriate timing of their administration, and demonstrate the ability to safely prescribe medications for alcohol use disorder (AUD) in carceral settings.
    5. Determine counseling strategies for discussing MAUD and other AUD treatments with patients.
    6. Identify effective collaborative approaches when working with interdisciplinary teams to develop and deliver comprehensive treatment plans as well as address contextual challenges of alcohol use disorder in carceral settings.
